Note: PCP is an abbreviation for Primary Care Provider.
Machine Readable JSON Provider File – Machine-readable formatting provides an electronic version of a document so it can be read or deciphered by a computer. All machine-readable documents are formatted with similar coding and provide a universal method for a computer to "read" documents. NMHC is required to post some of our documents in machine-readable format. Because of how they appear, machine-readable formats are typically not easy for humans to read.

Selecting a Primary Care Practitioner (PCP)
When selecting a PCP from our online provider directory, it is important that you contact the office of your preferred provider and ensure that he or she is contracted with NMHC and is available to add you as a patient. You will need to request an appointment with the provider to establish yourself as a new patient.
